Aloha from Gary "Kiki" Takashiro Blum, M.D.
Born and raised in Honolulu, Hawai‘i, Gary "Kiki" Takashiro Blum graduated from Roosevelt High School in 1993. He earned his undergraduate degree from the University of Michigan with highest distinction in 1996, and subsequently returned home to the University of Hawai'i for Medical School and Orthopedic Surgery residency.
He completed a one-year research fellowship during the course of residency, and has presented his research at numerous local and national orthopedic meetings. Following residency, he traveled to San Antonio, Texas, where he completed a one-year fellowship in Hand, Upper Extremity, and reconstructive Microsurgery. He was blessed to have worked with Dr. David Green and Dr. William C. Pederson, both giants in their respective fields.
Dr. Blum has spent time as a visiting resident and fellow at several programs across the country, including Harborview Medical Center in Seattle, WA and Mayo Clinic in Rochester, MN. He also had the good fortune of traveling internationally through orthopedic outreach programs; first to the South Pacific (Fiji, Western & American Samoa) with Shriners Hospital for Children, then Vietnam (Saigon, Hue, and Hanoi) with Orthopedics Overseas and the American Academy for Surgery of the Hand.
Dr. Blum, his wife Shawn and their four children Siana, Brady, Kaya, and Bella are thrilled to be home in Hawai‘i with family and friends. He enjoys trail running, hiking with his family and soaking up the beauty of Hawai‘i. He has been coaching youth soccer, basketball, and volleyball from 2012-present.
He and Dr. Darryl Kan opened All Access Ortho on O‘ahu in 2013 as the first (and still only) dedicated Orthopedic Urgent Care. They have since expanded to three locations: Hale Pawa‘a, Mililani, and Kapolei.

Brittni Kimie Peralto Hoe, PA-C
Maui native Brittni Kimie Peralto Hoe is Lead Physician Assistant and Clinic Manager at AAOM. In fact, it was her return home in 2020 that inspired the expansion to the Valley Isle.
Brittni graduated from Kamehameha Schools, Kapālama Campus, in 2003. She went on to earn a B.S. in Natural Science Studies at Loyola Marymount University and an M.S. in Physician Assistant Studies at Touro University. After becoming nationally certified as a Physician Assistant in 2010, Brittni worked at The Everett Clinic in Washington State, in the specialty of Hand Orthopedics and Plastic Surgery, for several years. She returned home to Hawai‘i in 2013, and started her employment with AAO on O‘ahu the following year. Over the next eight years, Brittni helped to expand AAO to two additional O‘ahu offices and was promoted to Lead Physician Assistant, overseeing her fellow PAs and staff across all office locations.
In 2020, she and her family relocated to Maui, which motivated the opening of the new office. Brittni, her husband and their two children are thrilled to be here and to establish strong roots as a family. Born on Maui, Brittni feels it is her kuleana to help community members by providing them with direct access to specialized orthopedic care. In her time outside work, she enjoys yoga, travel, the beautiful beaches of Maui, and spending quality time with family and friends.

Adam Mays, PA-C, CAQOS
Adam Mays began his career in medicine in 1989 as a U.S. Navy Corpsman during the era of Operation Desert Storm aboard a thousand-bed floating hospital. He went on to gain experience across multiple surgical specialties before continuing his training in orthopedic medicine and its surgical sub-specialties.
Adam is currently one of the only Physician Assistant providers in the State of Hawai‘i with Certified Added Qualification in Orthopedic Surgery. He was the first to be certified in Washington State, where he completed the MEDEX Northwest program at University of Washington School of Medicine.
Working in private practice orthopedics in the outer Seattle area, Adam also served as a lead faculty preceptor at a physician residency training program, teaching musculoskeletal medicine.
He has extensive experience in clinical, procedural and surgical orthopedics, with expertise in diagnostic imaging interpretation, determining surgical indications, and fast-tracking surgical intervention, when appropriate. In his spare time, Adam enjoys sports from snowboarding to golf, and has completed several Ironman races along with other endurance events.

Pediatric Orthopedics Specialty Consultation Clinic
Byron Izuka, M.D.
AAOM offers Pediatric Orthopedics Specialty Consultation Clinic with Dr. Byron Izuka, a Board Certified Pediatric Orthopedic Surgeon. Dr. Izuka has more than 20 years of experience in the field and is well-published. Dr. Brian will be visiting our Maui clinic for consultations the third Thursday of each month.
Dr. Izuka is a proud graduate of ‘Aiea High School. He received his undergraduate degree from the University of California at Irvine and attended the University of Hawai‘i John A. Burns School of Medicine (UH JABSOM) and Orthopedic Surgery Residency Program. He then completed a fellowship in Pediatric Orthopedic Surgery at the Nemours Children's Clinic in Orlando, Fla., before returning home to stay.
Dr. Izuka is a UH JABSOM Associate Clinical Professor and sees patients with orthopedic problems of all ages, with particular expertise in newborns, infants, children and adolescents. He has developed several novel treatments for fractures in children (what is done for adults may not work or even be necessary in children) with numerous presentations, publications and research awards to his credit. He also believes in giving back to the community and has volunteered as a sideline team physician for the Aiea High School and University of Hawai‘i athletic departments since 2001.
In his spare time, Dr. Izuka enjoys discovering new ethnic foods to try (maybe too much!), outdoor activities and all things Disney.

Patients have even more specialized options for care through AAOM’s partnership with Hawaiʻi Hand & Rehabilitation Services. Co-owners Sheri Azama and Tammy Momohara are both Certified Hand Therapists who formed HHRS in 2008. They have several locations on O‘ahu to meet the needs of patients requiring specialized care to manage hand conditions and are eager to help meet the needs of patients on Maui as well. HHRS provides onsite Hand Therapy and fabrication of custom orthotics for patients who need these services, whether due to chronic conditions or injury, or before or after surgery. Sheri Azama
Occupational Therapist and Co-Owner of Hawaii Hand & Rehabilitation Services
Sheri graduated with her Doctorate in Occupational Therapy (OTD) from Creighton University in 2002. She has a passion for hand therapy and obtained her Certified Hand Therapy (CHT) credential in 2010 and her Certification in Lymphedema Therapy of the Upper Extremity (CLT-UE) in 2019.
When Sheri is not working or being an Uber driver for her 2 kids, she enjoys spending time with her family and friends, traveling, working out at Pure Barre, boba drinks, and watching Netflix and K- Dramas with her beloved dog, Bella.
Tammy Momohara
Occupational Therapist and Co-Owner of Hawaii Hand & Rehabilitation Services
Tammy obtained her Occupational Therapy (BS) degree from the University of Puget Sound back when perms and cassette players were the rage. Following her initial calling to the profession in pediatrics, she found a renewed passion for OT in hand therapy. She earned her CHT credential in 2005.
In her free time Tammy enjoys the outdoors: playing in the mud (hiking), feeling the wind in her face (bicycling), carrying weight on her shoulders for many miles (backpacking) and most recently, hitting a hole-filled plastic ball (pickleball).
